Individualized Supervised Practice Pathways (ISPPs)

The Nutrition Science Department is participating in the Individualized Supervised Practice Pathways (ISPPs) established by the Academy of Nutrition and Dietetics through the Accreditation Council for Education in Nutrition and Dietetics (ACEND). Sage has been approved to accept 4 ISPPs interns each year. Before you apply for an ISPP at Sage, you should:

  • Have a GPA of 3.0 or above
  • Have applied to a dietetic internship through DICAS and failed to be matched
  • Plan to complete your rotations outside the Capital District of New York State
  • Secure all potential rotations and placements for the internship. A list of all the preceptors’ names and information with confirmation is required before beginning the program
  • Set your own start date and number of hours you plan to dedicate to the internship (Note that these hours cannot be paid hours and should be on your own time. For example, if you are a diet technician at a hospital, you may decide to work Friday and Saturday and receive pay for these hours.  You may then plan your ISPP clinical rotation with the dietitian from Monday through Thursday for 10-hour days without compensation. The paid hours may not count toward your ISPP hours.

Cost of ISPP at Sage

Beginning October 2012, the cost of ISPP at Sage is $6,250 for the entire program, paid in full prior to the start of the program.

Additional expenses such as physical examination and immunizations, professional liability and personal health insurance, required reference books, photocopying or paper and printer cartridges for presentations and poster sessions, and a full length lab coat are the responsibility of the ISPP applicant. In addition, travel (gas, tolls and/or parking) to facilities and for field trips, if any, must also be budgeted. Projected cost for these incidentals is $3000.00.

How to Apply to the Sage ISPP

The Sage ISPP has open enrollment with no application deadline. Acceptance depends on availability of slots. For information on availability, contact Dr. AbuSabha via email at abusar@sage.edu. 

To apply,

1. Send your complete DICAS application to: Rayane AbuSabha, PhD, RD, The Sage Colleges, 65 First Street, Troy, NY 12180.  

2. Contact Dr. A within 2 weeks of mailing your application to ensure its receipt and time when acceptance/rejection decision will be finalized.

The decision for acceptance is based on: 1) strength of the DICAS application, 2) GPA, 3) secured rotations (especially the clinical MNT rotation).

 

ONCE ACCEPTED INTO THE SAGE ISPP YOU NEED TO:

1. Contact Dr. AbuSabha

Contact Dr. A to receive the ISPP Information Document, the Medical forms and information documents, a generic facility contract, as well as the ISPP Handbooks.

2. Report Your ISPP Rotations Schedule

Complete your rotation schedule BEFORE beginning your ISPP experience. Use the following link to complete your initial ISPP rotation schedule, and while in the ISPP program, use this link to update any changes to your schedule:

3. Provide Facility and Preceptor Information

Before starting each rotation, submit to Dr. A the following: 1) A signed contract with each facility you will be using, and 2) the resume for each preceptor who will be mentoring you during your rotations.

Note: Only one contract is needed per facility (for example you are completing MNT, FSM, LTC and outpatient at one facility, only one contract needs to be drafted). Similarly, only one resume is needed per preceptor/mentor.

 

Evaluation Forms

The following are the evaluations to be used with the Sage ISPP Program:

Intern Clinical Performance Evaluation -

1) Preceptor uses this form to evaluate the intern's performance in clinical rotations (acute care, long term care and outpatient)

2) Intern uses this form to evaluate own performance in clinical rotations (acute care, long term care and outpatient). Intern only complete the form during rotations that are 4 weeks or longer. During these rotations complete this form only once at mid-rotation to use it as a self-evaluation of progress.

3) This form is to be completed once at the end of rotations that are shorter than 4 weeks, and twice (at mid-rotation and end-of-rotation) during rotations that are 4 weeks or longer

Intern Management/Community Performance Evaluation -

1) Preceptor uses this form to evaluate the intern's performance in food service and management, wellness, nutrition education, school and community rotations

2) Intern uses this form to evaluate own performance in food service and management, wellness, nutrition education, school and community rotations. Intern only complete the form during rotations that are 4 weeks or longer. During these rotations complete this form only once at mid-rotation to use it as a self-evaluation of progress.

3) This form is to be completed once at the end of rotations that are shorter than 4 weeks, and twice (at mid-rotation and end-of-rotation) during rotations that are 4 weeks or longer

Practicum Hours Verification Form - Intern completes this form on a weekly basis to report total number of hours completed each week

Clinical Rotation Checklist - Intern uses this form as a checklist to ensure that specific DI clinical requirements have been met. Form is to be completed at the end of each clinical, LTC, and community rotations

Evaluation of Oral Case Study/Journal Presentation - Preceptor and other attendees are to complete this form evaluating intern's presentation on the final case study and on the three journal presentations

Evaluation for Diet Education - Preceptor observes intern during diet education and counseling and rates intern performance using this form. A minimum of three forms should be completed

Intern Evaluation of Preceptor and Facility - This form is to be completed by the intern once at the end of each rotation

Intern Evaluation of Rotation- This form is to be completed by the intern once at the end of each rotation
