Application + Appointment Process for Fall 2010

Materials may be submitted either through a college advisor or directly from an individual student, but all materials must be submitted in one package. Electronic submission of the waiver portfolio or the application packet will not be accepted.

All applicants to Dietetic Internships and Pre-professional Practice Programs must participate in computer matching. Applicants should request instructions and a mark/sense card to prioritize their DI and/or AP4 preferences. Applicants should request this material from any ADA-accredited Didactic Program in Dietetics or from D&D Digital. This request should be made to allow turnaround time for submitting by a February 16, 2010 postmark deadline. There is no charge for this material. However, there is a $50.00 charge for computer matching that is due with the applicant's prioritized ranking.

There are new web-based features for applicants participating in the computer matching process. Applicants can either download an application form or register online at http://www.dnddigital.com/ and enter contact information, Dietetic Internship preferences and pay the $50.00 fee with a credit card on or before the deadline date.

Several forms and documents are required for the Dietetic Internship Application. Please review this section carefully to ensure all forms and required documents with corresponding copies are included in the application packet.

1. SGS DI Release Form: complete one copy of this form.

2. Application form: submit a typed and signed, original application form printed from The Sage Colleges web site, plus two copies. The ADA application [doc] must be used. **Please note, the SGS DI does consider work experience greater than the five years. Applicants with more than five years of work experience are encouraged to include this information on the application form.

A check for $40.00 payable to The Sage Colleges must accompany each application. This fee is waived for current Russell Sage College students and graduates. Only one application fee is required if applying to both the Dietetic Internship and Sage Graduate School.

3. ADA Waiver+ Recommendation: three recommendations with the waiver cover page and original signatures are required plus two copies of each, completed by the following individuals:

  • A professor in food science, nutrition or food service management;
  • A work or a practicum supervisor; and
  • Either a second professor in foods/nutrition or a related field or a second work/practicum supervisor. Use the ADA Recommendation Form. ADA Waiver and Recommendation Form [doc].

4. Clinical Observation Document: complete one copy of this form.

5. One copy of official transcript(s), i.e., in a sealed envelope, complete through first semester senior year from all colleges/universities where courses have been taken to complete DPD and BS requirements. If transcripts do not show descriptive titles, then a catalog description must be provided.

6. Verification of completion of DPD requirements or official declaration of intent to complete ADA/DPD requirements.

7. A single spaced typed letter addressed to the DI Director, of no more than two pages in length, using 1 inch margins, plus two copies. It is to include the following: reason for applying to the Sage Graduate School Dietetic Internship: personal and professional achievements, short and long term goals, and strengths and weaknesses.

8. If also applying to a MS degree program, the entire packet will be forwarded to Sage Graduate School after the internship review process is concluded. An additional application fee is not required. The DI Recommendation Forms may also be used in place of the Graduate School references. A Graduate Record Examination (GRE) is not required.

Check the box for Dietetic Internship (DI Certificate). If applying for enrollment into a MS degree program, be sure to also check the corresponding box.

Application packets must be postmarked or hand delivered no later than February 17, 2010. Do not place applications in binders, folder packets, plastic covers, etc. Send or hand deliver applications to:

Applicants will not be notified if the packet is incomplete. You are urged to carefully compile the application packet. Incomplete packets will not be reviewed. Packets from applicants who do not meet the minimum requirements, i.e. minimum GPA of 2.75 overall and 3.00 for DPD courses, will not be reviewed. Application materials will be returned to applicants upon receipt of a written request by May 15, 2009 with current address and a $10.00 shipping and handling fee. Acceptance Process

1. Applicant Notification Day for the Computer Match process is April 5, 2010 via email from D&D Digital Systems. The Sage Graduate School Dietetic Internship is not allowed to notify students prior to this date. Students must accept/reject the computer match within 24 hours of Notification Day.

2. Applicant Appointment Day: For the Sage Graduate School Dietetic Internship (SGSDI), applicants are to telephone and email the Program Director by 5:00 pm on April 7, 2010, to confirm the acceptance or rejection of the match with the Sage Graduate School Dietetic Internship program (518) 244-2075, bellcm@sage.edu). The Sage Graduate School Dietetic Internship forwards a confirmation acceptance letter on or about April 10, 2010. Additional program information and directions pertaining to enrollment into the SGS DI are forwarded to each new intern at that time, as well as throughout the summer.
