Articulation Agreement between Siena College and Sage Graduate School Physical Therapy Program (7/9/07)
Terms of the Agreement: Physical Therapy Admission
Siena College students who demonstrate the following will be guaranteed admission to Sage Graduate School for the DPT degree:
- Earned baccalaureate degree (or in progress)
- Minimum overall 3.0 GPA
- Minimum science GPA of 3.0 (Anatomy & Physiology I and II; Physics I & II; Chemistry I & II, Cell Biology)
- Completion of Physical Therapy Program prerequisite courses with grades of C or better (see prerequisite courses listed below)
To guarantee placement in the DPT Program students agree to submit the following information: By June 1 of the year preceding graduation from Siena College:
- Completed SGS application packet (fee waived)
- Official transcripts from all college institutions attended
- Current resume
- One letter of recommendation from the Siena College health advisor or equivalent faculty representative
- List of all outstanding prerequisite courses with an expected date of completion
By October 15 of the graduation year at Siena College:
- Clinical Observation Document: 40 hours of clinical observation with a physical therapist

- Enrollment deposit of $50 (non-refundable, though to be applied toward graduate tuition)

Upon graduation from Siena College:
- Official transcripts documenting completion of undergraduate degree and PT Program prerequisite courses
