Financial Aid Forms and Notifications
This webpage gives you access to Financial Aid Forms and information regarding any missing documentation the Financial Aid Office needs in order to process your financial aid for the 2016-17 year. PLEASE NOTE: You can obtain the missing documents by logging into your SageAdvisor account, clicking MY SAGE AID and reviewing the required documents tab. Please take note of the Form Number, so you can easily find your required information. If you are required to print a form but would prefer to have the form mailed to your home address, please contact the financial aid office at (518) 292-1783 in Albany or (518) 244-4525 in Troy.
You may submit your required documents by mail, fax, email or in person. Please be sure that your name and student ID are on each page of the documentation you submit.

If you were selected for Verification, you must submit a verification worksheet along with documentation to support the information you/your parent(s) provided on your FAFSA. Your forms can be submitted to the office by fax (518) 244-2460, email FAverification@sage.edu, or mailed to the Troy address above, Attn: Megan Henry.
