As a student seeking tutorial support, I understand and agree to the following responsibilities:

• Will be prepared for the tutoring session and bring the necessary text book, or reading material, lecture notes, and documents;
• No use of mobile phone during sessions (unless otherwise permitted);
• Attempt the given assignment before the session;
• Prepare questions and/or identify specific problem areas;
• Honor the allotted appointment by arriving on time, and will not exceed the maximum daily tutoring limit set per subject and day.

Contact the Academic Support Center, in advance, if I should have to miss a scheduled session.
