Application Instructions and Procedures
The Sage Graduate School uses a student-managed application procedure. Your application, transcripts and letters of recommendation should be submitted in a single envelope. This will allow Sage Graduate School to process your application and to make decisions in a more timely manner.
Application Procedures for Admission to Degree Study:
(depending on the program, there may be additional requirements; see academic programs for more details)
1. Application

Please note: There are supplemental forms required for each of the four schools:
- School of Nursing and Health Sciences
- School of Arts and Sciences
- School of Education
- School of Professional Studies
Please complete both sides of the application form and sign before returning to Sage Graduate School. Click here to Apply Online.
2. Application Fee
The $40 application fee must be a check or money order made payable to "The Sage Colleges". DO NOT SEND CASH. The application fee is waived for the following people:
- any graduate of one of The Sage Colleges;
- any student applying as part of The Collaborative Agreement in Teacher Education between Siena and Sage;
- any student who works for companies that participate in the corporate reimbursement program.
3. Transcripts

Send the registrar of each college or university which you attended (whether you earned a degree or not from that institution) a transcript request form and one of the transcript envelopes. Instruct each one to enclose your transcript in the envelope. Request the registrar(s) to sign across the seal and to mail the envelope(s) to you. DO NOT BREAK THE SEAL. If we receive a transcript that has been opened, the contents will not be accepted. If you need more than two transcript envelopes, please prepare them yourself. Only one copy of each transcript is required. Please be sure to list all colleges and/or universities on the application form. If you have attended any of The Sage Colleges, these transcripts will be requested internally and do not have to be requested by you. If you are a graduate of one of The Sage Colleges, we will retain all of your records from the schools that you have previously attended**. If you have attended classes or any institution after you graduated from any of the Sage Colleges, those transcripts must be requested by you. **If you graduated more than seven years ago from one of The Sage Colleges, you will need to request transcripts from all other institutions. 4. Recommendations

Give each person preparing your recommendation a form and an envelope. Request each to enclose the completed recommendation form in the envelope, seal it, and mail the envelope to you. DO NOT BREAK THE SEAL. If we receive a recommendation that has been opened, the contents will not be accepted. Two recommendations are required for admission. If you are a student applying for The Collaborative Agreement Collaborative Agreement Recommendation Form in Teacher Education the following rules apply:
- Students who have completed the Secondary Education Certificate program in good standing do not need to submit a letter of recommendation.
- Students who complete degrees in majors other than Secondary Education must submit a letter of recommendation from their faculty advisor (see enclosed form). Please give the academic advisor who is preparing the letter of recommendation the enclosed form entitled "The Collaborative Agreement in Teacher Education, letter of Recommendation Form" and envelope that corresponds to it. Request that the advisor enclose the completed recommendation form in the envelope provided, seal it, and mail the envelope to you. DO NOT BREAK THE SEAL. If we receive a letter of recommendation that has been opened, the contents will not be accepted.
5. Career Goals Essay
Write a one to two page typed statement of your personal and professional goals as they relate to your graduate studies. This requirement is waived for those students applying to The Collaborative Agreement in Teacher Education.
6. Current Resume
Enclose a copy of your current resume. This requirement is waived for those students applying to The Collaborative Agreement in Teacher Education.
7. International Applicants
In addition to the above required, international applicants must submit proof of English proficiency which could be results from an official TOEFL report indicating a score of 550 (paper), 213 (computer based score) or 79 (internet based score) or above. Students who have graduated from a US College or University may be waived from submitting the TOEFL. An official academic transcript evaluation from the World Education Testing Service, and a Financial Affidavit documenting financial support is also required.
8. Occupational Therapy Applicants
The occupational therapy application process includes a career goals essay, a resume, transcripts and completed application, and the completion of the following forms: Nursing and Health Sciences Supplemental Forms
9. Physical Therapy Applicants
In addition to the career goals essay, a resume, transcripts and the application, please submit both of the following forms. Nursing and Health Sciences Supplemental Forms
10. Master of Arts In Teaching Applicants
In addition to 2 letters of recommendation, a career goals essay, a resume, transcripts and the application, an interview with the faculty Program Director is required. Every applicant will have to complete a written sample assessment. This is a fall entrance program only and the application deadline is April 1st. Education Supplemental Forms
Note: If a recommendor or registrar refuses to send a recommendation or transcript to you, have him/her send it directly to: Office of Admission, Sage Graduate School, 45 Ferry Street, Troy, NY 12180. Please make a prominent note on your application if we are to receive credentials separately.
