Distance Applicant Procedures
Practicum placements for all rotations are the sole responsibility of the intern and must be confirmed upon application to the program.
The Distance Track Preceptor Application and Dietetic Internship Affiliation Agreement(s) must be submitted and fully executed for each site prior to the start of internship. These forms are sent to the interns upon acceptance to the program and available on our website. Please do not have these completed and mailed to us. Should you be accepted into the program the DI Director will advise you when these must be completed.
Accepted Students
After acceptance into the Russell Sage College Distance Dietetic Internship program and prior to the start of the internship, ALL preceptors must complete Russell Sage College Dietetic Internship Distance Track Preceptor Sign-up Form and submit two signed copies of the Russell Sage College Dietetic Internship Agreement below:
Note that there are 3 different contracts: “Clinical”, “Non-Clinical” and “Inclusive”. Please read the definitions to identify appropriate contract, as well as if the facility is a “For Profit” or “Not-For-Profit/Government Agency”.
