Immunization Forms

Please review this information carefully.

All students taking six or more credit hours are required to complete the Student Immunization Form. If you submitted your records already, you do not need to resubmit them.

New Students must submit proof of immunization records by August 1 for the fall semester and December 1 for the spring semester. 

Be advised that uploading false or forged documentation is a violation of College policy and subject to disciplinary action.
