    Please review this information carefully.

    All students taking six or more credit hours are required to complete the Student Immunization Form. If you submitted your records already, you do not need to resubmit them.

    We utilize a secure, user-friendly web portal for submission of your immunization requirements. This system is both HIPAA and FERPA compliant. Be advised that uploading false or forged documentation is a violation of College policy and subject to disciplinary action.

    The deadlines for submission are August 1 for fall semester and December 1 for spring semester. You must have an issued Russell Sage College Student ID and Sage email to submit documents. Instructions follow.

    From any browser, go to https://sage.studenthealthportal.com/

    • Enter your Sage.edu email address and your current password you would use to access a computer on campus.
    • Once logged into your new web portal, navigate to the Document Upload Tab.
    • Select on the drop down arrow under Document Type and select CDC Issued Vaccination Card (to submit your COVID-19 Vaccination Record)or Immunization Records to submit any other immunization forms.
    • Select Browse to search for the image file of your immunizations or CDC-issued Vaccination Card (You will have to save a copy of this card to your computer before selecting it in Browse).
    • Once you have the Image Type and Image selected, Click Save. This will send your document to the Wellness Center.

    Do this for each Image Type available in the Image Type dropdown menu in your Document Upload Area.
