Immunization Requirements

Please review this information carefully.

All students taking six or more credit hours are required to complete the Student Immunization Form. If you submitted your records already, you do not need to resubmit them.

As of December 15th, 2021, Russell Sage College has mandated the Covid-19 Booster.

Residential Students: Must show proof of vaccination booster prior to return to on-campus housing or within thirty days of becoming eligible for a booster.* Commuter Students: Must show proof of a vaccination booster by January 30 or within thirty days of becoming eligible for a booster.* Students who do not visit campus for academic or recreational purposes are not required to submit proof of vaccination.

New Students must submit proof of immunization records and their CDC issued vaccination card by August 1 for the fall semester and December 1 for the spring semester. You must have an issued Russell Sage College Student ID and Sage email to submit documents. 

Be advised that uploading false or forged documentation is a violation of College policy and subject to disciplinary action.
