Dear Russell Sage College Student,
All students taking six or more credit hours are required to complete the Student Immunization Form. Please review the information on this page very carefully.
We are now utilizing Med+Proctor for verification of your immunization requirements (if you submitted your records already, you do not need to resubmit them). This system is a cloud-based, fully automated and user-friendly platform that is both HIPAA and FERPA compliant.
Med+Proctor will send an invitation to your Sage email to set up your account with your student ID number. This is free, but there is an optional one-time $10 set up fee that comes with a lifetime access to your records, if you so choose.
You will log on to Med+Proctor using the instructions below. You can print the medical forms and take them to your medical provider or obtain them from your medical provider, former school, former employer, etc. Be sure all forms are filled out in their entirety, including dates and signatures. Then you simply upload your forms to the Med+Proctor site. All of your documents must be uploaded to Med+Proctor. With this format, you can even upload clear pictures straight from your smartphone using an app such as Genius Scan. Do not bring, fax, mail or email any medical forms to the Wellness Center.
Please note that the deadlines for submission are August 1 for the fall semester and December 1 for the spring semester.
